T 5 Pilot bushing slap worn out why?

Engine: 305 stock / ZZ4 AFR 195 9.7:1
Transmission: T5 / t10 / Jerico
Axle/Gears: 10blt w 3.42, 9 in w /3.80 DL
T 5 Pilot bushing slap worn out why?
I blew a clutch at an open track day. Pulled the clutch out and went to install the new unit and the alignment tool was very loose.
Checked the piolt bushing and it was oversized enough to stick the big end of a 3/8 socket extension inside of it. It would swallow a 5/8 bolt.
I even used the extension to pack the grease to back out the bushing.
I put it back together with new bushing as I needed to get the car moving. I did not have time to do a post mordum.
Why would the bushing wear this much and was this what destroyed the clutch?
I do not want any more transmission removal experience ant time soon. It is not fun on your back putting a T 5 back in.
